Step 1 — Complete Payment

Step 2 — Welcome Email with Scheduling Link
A welcome email is sent automatically with full instructions and a private link to schedule your appointment at your convenience through our online calendar.
Step 3 — MedFiller Pre-Intake (triggered on email open)
Once you open your welcome email, a personalized MedFiller link is automatically sent to you. This lets you complete your portion of Form I-693 before your appointment — saving you time in office.
Step 4 — Your Exam Appointment
Arrive at your scheduled time with all required documents. Your Civil Surgeon will complete the full medical review, physical exam, vaccination review, and required lab work.
Step 5 — Sealed I-693 Package (~1 Week)
Within approximately one week, your completed and officially sealed Form I-693 will be ready to submit directly to USCIS.

💉 Vaccine Requirements
- Required per CDC age-based guidelines
- May include MMR, Tdap/Td, Varicella, Hepatitis A & B
- Polio, Influenza (seasonal), and others as applicable
- See Form I-693 Page 12 for the complete required vaccine list
- Missing vaccines must be obtained through the Dept. of Health, your PCP, or a pharmacy
- Vaccines are not included in the exam fee
🔬 Required Laboratory Tests

- ✓ We do not draw labs for children in our office
- ✓ A lab order will be provided for an outside lab
⚠ Important Fee Notice
The exam fee ($350 adults / $300 children) does not include laboratory tests or vaccines. Please plan for additional costs. In-office lab fees are typically $85–$105. Vaccine costs vary and must be obtained separately if you are missing any required immunizations.
Government-Issued Photo ID
Driver's license or passport required for all adult applicants.
Birth Certificate (Children)
For children without a passport. Another government-issued ID is also acceptable.
Vaccination Records
All immunization history. Records must be translated into English if in another language.
Translated Documents
Any records not originally in English must be accompanied by a certified English translation.

It is not the Civil Surgeon's responsibility to complete the applicant's portion of Form I-693 or to obtain vaccination records on your behalf. You will receive a MedFiller email link to complete your portion before your appointment.
